  • Shock threshold SBP: 5th centile ≈ 70 + (2 × age in years) mmHg for 0–10y; <90 mmHg at >10y.
  • SpO2: ≥95% on air normal; 92–94% concerning; <92% serious; any supplemental O2 raises the bar.
  • Temperature: fever ≥38.0°C (red flag at any age <3 months per NICE CG160). Hypothermia <36.0°C.
  • CRT: <2s normal; 2–3s borderline; ≥3s delayed / concerning.
  • AVPU: A normal; V = concerning; P/U = emergency (GCS equivalent ≤8).

Based on APLS 6th edition age-banded normals and NICE CG160 / CG98 red-flag thresholds. Trust PEWS charts differ slightly - always follow local guidance. This tool is decision support, not a substitute for clinical judgement.
